In order to apply for a placard, obtain a Handicap/Disabled form from the County Treasurer's Office or print a form from www.ksrevenue.org, form TR 159, and take it to your doctor for completion.  Return the signed application to the Treasurer's Office or mail to the Franklin County Treasurer, 315 S. Main, Ottawa, KS 66067.  The placard will then be mailed to you, or you may bring the application to the office and be given the placard at that time, at no fee.

When applying for a plate, obtain a Handicap/Disabled form from the County Treasurer's Office or print a form from www.ksrevenue.org, form TR 159, and take it to your doctor for completion.  Request for a disabled license plate must be made in person.  Present the license plate registered to your vehicle, your current registration, and proof of current Kansas Liability Insurance.  Your current registration plate will then be surrendered in exchange for a disabled plate.  The fee is $5.00.

Each disabled person is allowed to have one disabled plate and one placard, (or) two disabled placards.
